New Patient Registration

You can register with the practice online by completing this registration form

Please be advised that your registration will be complete immediately in over 95% of cases. If there are queries, you will be contacted by one of our team within 5 working days to resolve everything in order to get your registration completed.  

You should also download the NHS app at the same time as this is the first place you will get notifications from the practice and be able to view any results and new notes. www.nhs.uk/nhs-app 

If you prefer to fill out on paper, please download a printable form (PDF, 161KB) or alternatively you can visit the surgery for us to print one for you.

New Patient Notice

If you live outside of our catchment area you will not be able to register with the practice.

Everyone is welcome in general practice – lack of ID will not stop you registering at our practice. We ask for ID if you would like online services but this is an optional element of your registration, and if you have no ID, we can complete additional checks with yourself in order to get this part set up. You DO NOT NEED to have ID - so please don't feel lack of this will cause a problem.

You can also register with the practice online through Patient Access. You will need to contact reception first to signup for this service.

If you are struggling to do this online, please call into the surgery where the reception team will give you a paper copy of the form for you to fill out and return. Should you require assistance with this, one of the team will be happy to help. We are aware that our patients have differing accessibility requirements and we are dedicated to providing a high level of service to all of our community.

Please try to fill out as much of the form as possible. If you leave out important pieces of information there may be a delay in your registration.

Please note as part of addressing health inequalities within different parts of our community, we ask some personal information regarding your protected characteristics to help us know our patient population. All of these include the option for you to not give this information – so if you feel that this is something you’d prefer not to answer, we have included the option to select “prefer not to say”.

If you are taking regular medication, it may be a good idea to obtain a copy of your full prescription from your previous surgery before you register with the practice. This will mean we have proof of your medications while we are still waiting for your records from your previous surgery.

Named Accountable GP

All patients at New Islington Medical Practice have been allocated to Dr W Ali who is responsible for patients’ overall care at the practice and is your named GP, however in the event that a patient expresses a wish to change their named GP we are happy to accommodate this request and would ask that you contact a member of our Reception Team who will be happy to arrange this for you.

Families

Please feel free to register any family members who are not already registered here. You can do this by filling out another registration form for each individual. We welcome families!

Please note ALL under 16s must be have their primary caregiver registered at the same GP practice - this is a Manchester wide policy which helps us safeguard our vulnerable patients.

Controlled Drugs

Please see our practice policy regarding controlled medications (DOCX, 31KB) that are considered to be high risk.

Please note that if you are currently prescribed any of the drugs listed, you will need a review with one of our clinicians as we will not prescribe these drugs on a long term basis.

 

Amber Drugs

Please note there are some drugs that are labelled Amber - in that they should not be initiated or prescribed by primary care clinicians in the absence of a shared care policy. Our practice policy is that we will only accept shared care agreements with an NHS provider. 

Please note that if you are currently prescribed any of the drugs that fall into this category, you will need a review with one of our clinicians as we will not prescribe these drugs on a long term basis.
